Introduction

Weight Management is part of the NHS England Enhanced Service. It assists practices in identifying patients living with obesity and subsequently referring them to local or national weight management programs.

 

This service focuses on patients with a BMI of 30 or above (or 27.5 for certain ethnic groups) and encourages brief, supportive conversations that lead to meaningful referrals. Practices are compensated per referral, and accurate coding is essential for monitoring and claiming activity.

 

The Weight Management Dashboard in Ardens Manager simplifies the process of identifying eligible patients, tracking referral activity, and keeping up with your practice's progress. This support article will walk you through using the dashboard to support this work.


Accessing the Dashboard

Log in to Ardens Manager.  

 

Select the Contracts icon, followed by Weight Management ES '25-26.



You can view your Weight Management data using the Overview tab or by using the Obesity tab. 



Clinical Activity

This section is organised into three categories:
  • Data Quality - Highlights potential issues with patient data.
  • Referrals - Allows users to track the number of weight referrals completed or declined.
  • Review - Highlights patient records needing a review, as they appear eligible but have not yet received a referral.



Organisation Activity

This section contains three reports that require manual entry by the practice. To add supporting evidence for the indicator, click on + Add.



A submission box will open, detailing the requirements for this task or indicator. You will see a space to enter your evidence, including a field for adding any files or attachments if needed.

 

To complete this task, you can either Mark as In Progress to edit your response before submitting. Once you are ready to submit, click on Mark as Complete.



Monitoring Performance 

You can monitor your progress and achievements with each indicator using the visual progress bar or the Achieved column, which details the number of patients identified in the report.



Once you select a report, you can view the corresponding patient list by clicking on the Patients tab. You can refine this list to show all patients, only those who have achieved the indicator, or those who are still outstanding. Additional insights are available, including patient demographics, staff-level data, and trend analysis. You can also benchmark your data against other practices within your PCN, provided the appropriate data sharing agreement is in place. Rules

Rules for all reports can be found using the Rules tab on the dashboard. 

 

Click on the report you want to view the rules for. A page will open to the right. This will detail the particulars of the search. To view any code sets included, click on the blue hyperlinks.



Rules can also be located when viewing a single report by clicking on the Rules tab.



Contract

The Contract tab offers a clear, user-friendly breakdown of each component of the Weight Management Service Contract, allowing you to view all contract details in one place.



The Payment section explains how the payment framework operates, including confirmation of the national point value. Clicking on the ‘i’ next to the point payments provides further details on how this type of payment framework functions. 



The Services Available section outlines the specifications for each domain of the Network DES, including payment thresholds where applicable. To explore this in more detail, click on Go to Section. There, you will find a comprehensive breakdown of the criteria for each domain and category, along with the required thresholds and point values for each indicator.

 

You can also access the rules for each report by clicking the information icon ‘i’ next to the report title.
