Introduction

Structured Medication Reviews (SMRs) play a key role in the NHS England Network Contract DES. They aim to ensure that patients, especially those with complex needs or at risk of medication-related harm, gain the maximum benefit from their medications. Trained pharmacists conduct these reviews to ensure each patient's medication is safe, effective, and suitable, while also minimising unnecessary prescriptions.

 

Recently, there has been more focus on reviewing medicines that can cause dependence or withdrawal, like antidepressants, opioids, and gabapentinoids. As a result, SMRs are becoming even more important in supporting patients' health and well-being.

 

The Network Contract DES Dashboard in Ardens Manager allows GP practices and PCNs to easily identify patients who may benefit from a review and to track progress. This support article will show you how to use the dashboard to support high-quality, patient-centered medication reviews within your organisation. 


Accessing the Dashboard

Log in to Ardens Manager.  

 

Select the Contracts icon, followed by Network Contract DES ‘25-26.

Click on the SMR tab. You can also view the reports within the "Overview" tab.

The dashboard will show your progress towards completing SMR reviews for eligible patients. A review is considered complete when all elements have been coded, including a renal function test, weight, and creatinine clearance.

 

The dashboard also provides a breakdown of SMRs completed by patient cohort. This includes patients prescribed potentially addictive medications, permanent care home residents, and patients with severe frailty. You can view the breakdown as a percentage of each cohort and the number of patients completed.



The Remaining column will detail the number of patients yet to receive an SMR out of the eligible cohort. 


To identify the list of patients of who not yet had their SMR, click on view next to the report title.


 Click on the Patients tab. 

To view only the remaining patients, filter your patient list to show remaining patients only using the drop-down menu. 

To review an individual patient record, click on the copy icon next to their name, and then paste this into the patient search function in your clinical system. 

You can export the full list of patients from Ardens Manager. Click on Export and choose your preferred format. You can export to CSV for bulk messaging with a third-party platform, or use the second option specific to your clinical system to import the patient list back into EMIS or SystmOne for further action. 

How to View Reports

To see more details of a report, click the View button next to the report name.


Once you select a report, you can view the corresponding patient list by clicking on the Patients tab. You can refine this list to show all patients, only those who have achieved the indicator, or those who are still outstanding. Additional insights are available, including patient demographics, staff-level data, and trend analysis. You can also benchmark your data against other practices within your PCN, provided the appropriate data sharing agreement is in place.
