Following the publication of the Network Contract DES 2022-23 performance and payments on CQRS, we have received a large number of queries regarding discrepancies between the data on CQRS and the data on Ardens EMIS, Ardens SystmOne and Ardens Manager.


We have produced the following CQRS discrepancies support article which we will continue to update if we have more information. The main reasons for any discrepancies that we are aware of at present are listed below.




1. Point discrepancies due to SMR-01A - EMIS Web practices only


Ardens have helped identify a potential issue with the EMIS Web GPES extraction for SMR-01A. We have raised this with NHS England who are currently investigating this with EMIS Web who have confirmed the issue and are implementing a fix. This could potentially result in a change in payment for EMIS Web practices. Further updates from NHS England will be distributed by commissioners following the completion of the investigation and fix. 



2. Point discrepancies due to EHCH-04


The EHCH-04 indicator looks at the mean number of patient contacts as part of a weekly care home round per resident, the numerator is calculated from GPAD data looking for national slot types of “Patient contact during Care Home Round” during the 22/23 financial year. The denominator is the number of care home beds allocated to the PCN by the ICB - this can be viewed within CQRS and will have a direct impact on the PCNs achievement and as such PCNs should check this value is correct. 


Guide-only reports were provided by Ardens looking for a coded entry indicating a ward round; while the Network Contract DES specification clearly outlined the requirement to use appropriately mapped slots for care home ward rounds. PCNs may be able to submit additional evidence (such as the number of coded entries) if they feel the achievement shown does not represent the activity undertaken.



3. Point discrepancies due to guide-only indicators


Some indicators (e.g. CVD-02, ES-01) are marked on Ardens EMIS, Ardens SystmOne, and Ardens Manager as being ‘Guide only’ reports. The reason for this is that Ardens are unable to build reports directly in line with the business rules due to not having access to the different data sources and also system limitations. Please see the Ardens EMIS Web and Ardens SystmOne disclaimers for more information.



4. Payment discrepancies due to indicator prevalence


IIF indicator payments are weighted by both practice list size and indicator prevalence. Indicator prevalence figures are only published on CQRS at the end of the financial year alongside overall performance. These values could not therefore have been incorporated into the Ardens Manager calculations throughout the year (to our knowledge no one was able to do this). Ardens highlight that the payment calculations should be used for predictive purposes only on the payment dashboard on Ardens Manager with a notice advising about prevalence weighting (if you click on the 'these calculations' link in the screenshot below)


For the Network Contract DES 2022-23 dashboard, Ardens will shortly be updating this with the end of year prevalence for all indicators which we are able to. This should mean that values are closer to the CQRS extraction values.  


For the Network Contract DES 2023-24 dashboard, Ardens will be generating their own national prevalence figures ahead of the financial year end which will be incorporated and continually updated throughout the year as we have more data. Whilst it will be impossible for the payment values to be 100% accurate, they should be significantly closer than not including any indicator prevalence.

For Ardens EMIS reports

  • The lastest 2022-23 version (5.32 Contracts - Network DES (22-23) - v.3.5) of the Ardens searches are installed
  • The audit report was run after the 31st March 2023 with a relative run date of 31st March 2023

 

For Ardens SystmOne reports

  • The reports were run after the 31st March 2023

 

For Ardens Manager dashboards

  • The correct data was uploaded onto Ardens Manager and the date next to each practice when benchmarking is 31/03/2023
  • All organisational activity values are correct; for example:
    • EHCH01d: Number of care home beds for which the PCN is paid the care home bed premium
    • EHCH-04: Mean number of patient contacts as part of weekly care home round per care home resident aged 18 years or over
    • AC-02: Emergency admissions for ambulatory care sensitive conditions
